Appointments

 Please call our office between the hours of  8:30AM and 5:00PM to schedule, cancel, reschedule or confirm an appointment. We schedule patient appointments between 8:00AM and 5:00PM Monday through Friday.

Many insurance plans require a referral from your primary care physician prior to approving specialty care. Please be sure that you are eligible for care prior to scheduling an appointment. This referral  policy applies to x-rays and any other procedures that require prior approval by the insurance provider. Comprehensive Orthopaedics advises potential clients to check with their primary care physician or insurance carrier for further information.

The nature of your orthopaedic problem

Referring physician information

Insurance information

Worker’s Compensation or auto accident information       if applicable

Were any tests performed that you will be bringing       with you?

What should I bring to my first appointment?

Your insurance card

X-rays, MRI or Cat Scan films or any test results      pertaining to your injury or illness

Copies of any pertinent medical records

A referral if required by your insurance carrier

Any required insurance co-pay

An interpreter if needed

Late Policy

If you arrive late for an appointment you may not be seen that day. “Squeezing” someone in only serves to shortchange the patient and the doctor, and it delays patients who arrive on time. We work hard to honor appointment times and respect patients’ schedules. For this reason, patients who come late are generally rescheduled. Patients who do not call and do not show up for appointments are charged a $25 “no show” fee.

Workers Compensation

You must provide all appointment billing information and have authorization from your carrier prior to treatment. If you do not have authorization, your appointment will be rescheduled.
