Insurance Plans

Comprehensive Orthopaedics currently participates with many health insurance plans, including those listed below. If you have questions regarding your plan's coverage, we advise you to contact your carrier or insurance representative directly for a complete list of benefits and services covered.

For the Record

• New insurance? New home? New name? Each time you schedule an appointment you will be asked for updated information.
• Insurances that require referrals must be in place with your insurance plan prior to treatment.
• There is a charge for Medical Record Requests and copies of X-Rays. Charges for copying are in accordance with State Provisions. Fees are invoiced before processing.

You must provide all appointment billing information and have authorization from your carrier prior to treatment. If you do not have authorization, your appointment will be rescheduled.
